The Public Health Agency of Canada is committed to protecting the privacy rights of individuals and safeguarding the personal information under its control in accordance with the Privacy Act. The personal information you provide will be collected by the Infectious Diseases and Vaccination Programs Branch for the National Vaccine Catalogue. IDVPB derives its authority to collect your personal information under the Public Health Agency of Canada Act.

Why are we collecting your personal information? By providing your email address, you hereby consent to received updates and notifications about the National Vaccine Catalogue.

Will we use or share your personal information for any other reason? Your email address will only be used to send notifications about the National Vaccine Catalogue. In some cases, information may be disclosed without your consent for purposes not outlined in this Statement pursuant to subsection 8(2) of the Privacy Act. The personal information collected will be retained and disposed of in accordance with the Department’s retention and disposal schedule.

What are your rights? You have the right to access and request a correction and/or notation to your personal information. You also have a right to complain to the Privacy Commissioner of Canada if you feel your personal information has been handled improperly.
